About this tool
A free, printable checkout sheet for shared classroom devices, library books, lab equipment, sports gear, and any other item that leaves your hands and needs to come back. Set your columns, pick blank rows or paste a student roster or item list, and the tool builds a clean sheet you can print on paper or save as a PDF. Everything runs in the browser — no accounts, no telemetry, no roster ever leaves your device.
Why an attributable loan changes behavior
Ask any teacher what they wish worked better with shared supplies, and the answer is almost always the same: pencils, calculators, and Chromebooks that go out and don't come back — or come back broken, and nobody is quite sure who had them. The fix is not a better supply cabinet. The fix is attribution: making sure that when an item leaves the cart, a specific name is written next to it.
Teachers online talk about this constantly. One high-school English teacher writes about a colleague who "doesn't let her students into class without showing her their pencil for class" — the item stays attached to a name. Others describe the "trade a shoe for a pencil" trick, or the shared-cart chromebook policy where a cracked screen is invisible because no one wrote down who had that device. When damage is anonymous, nobody pays for it — not the parent, not the district, and often not even in staff attention. When damage is attributable, most of the damage stops happening.
A paper sign-out sheet is the low-effort version of this. It doesn't require an inventory app, a district IT ticket, or a login. It just requires a clipboard and a pen. The whole ritual of writing your name next to the Chromebook you took is what does the work.
How to use it
- Type your sheet title (Chromebook Cart 12, Ms. Rivera's Class Library, Robotics Lab). Add a date and class/teacher if you want them on the header.
- Pick your columns. Item and Student are always on; toggle Out (date/time), In, and Condition. Add one custom column with a label of your choice — Serial, Room Number, Parent Signature, Cost, Return-by Date.
- Choose your row source:
- Blank rows — a clipboard-style sheet where you write in both the item and the borrower. Best when the item is the exception (one loose book, one calculator).
- Pre-filled students — paste your class roster; the Student column is filled in. Best when the same set of students is checking things out over a class period or unit.
- Pre-filled items — paste a list of items (Chromebook #01, Chromebook #02...); the Item column is filled in. Best for a device cart or library where each item has its own row and any student can sign next to whichever they took.
- Optional: toggle on Include a damage / incident log. A second table prints below the main sheet — item, description, cost, date, parent notified — for logging cracked screens, missing keys, or lost items.
- Click Build sheet. A live preview appears below.
- Click Print / Save as PDF to print, or Download CSV to keep a blank template.
The print stylesheet automatically hides the controls, navigation, and supporting copy so only the sheet prints. Row heights are set generously enough for a real signature but tight enough to fit around 26 rows on a US Letter page in portrait.
Where a paper checkout sheet earns its place
- Chromebook and iPad carts. One row per device with the device number pre-filled. Students sign next to the number they took. When a screen comes back cracked, the row tells you who had it and when.
- Class libraries and book rooms. Track which novel or leveled reader is with which student, and for how long — without needing to log into a library system for a class collection.
- Lab equipment. Microscopes, probeware, glassware, calculators, safety goggles — sign them out at the start of the period, sign them back in at the end. Missing at the bell? The list tells you where to look.
- Sports and athletics. Uniforms, balls, pads, keys to storage — coaches distribute at the start of a season and need to see them again in June.
- Music and drama. Instruments, scripts, costumes — items on multi-week loan where you need a paper record for the department budget.
- Makerspaces, career-tech, and shop classes. Hand tools, safety glasses, project bins — high-value or safety-critical gear where you must know who has what before dismissal.
- Front-office loaners. Spare chargers, translation devices, wheelchair, keys — anything the office loans out to a student, staff member, or visitor.
- Substitute teacher packets. A blank sheet in the sub folder means the sub can hand out and collect back materials even without your normal system.

Does any of this data leave my browser?
No. The generator runs entirely in your browser. Rosters, item lists, and notes are never sent, stored, or logged.
How many rows fit on one printed page?
About 26 rows on US Letter portrait with all five default columns. Switch to landscape for more rows or wider columns for long item names.
